MACON, Ga. (AP) -- Authorities say a Georgia inmate hanged himself in his isolation cell while two deputies and a nurse failed to follow inmate monitoring policy.
The Telegraph reports the issues surrounding the death of 40-year-old William Elder Homan are detailed in an internal affairs investigation by the Bibb County Sheriff's Office.
The 17-page investigative summary obtained by the newspaper this week says Homan killed himself in February, just days after he was jailed over a possible probation violation related to shoplifting.
The summary says investigators determined deputies Kaleb White and Geoffrey Osman doctored records that said they conducted required hourly inmate checks. Sheriff David Davis says the two have since resigned. The report says a Correct Health nurse named Crystal Wilson-Perez was fired for not following company's inmate monitoring policy.
