Unable to dispense paper towels at front hand basin by dish machine. Dish machine final surface rinse temperature at 142*F. Maintenance staff on site at time of inspection to repair machine. Dairy in reach in cooler holding at 49*F. Must be 41*F or below. Frozen food products soft to the touch in reach in freezer unit. Beverage in bulk tea urns not protected with lid. Wet wiping cloths stored on working surfaces and not in sanitizer solution. Floors dirty throughout prep areas. Recommend cleaning throughout service to maintain clean and orderly work areas.

Reasons including person in charge is not in control of several foodborne illness risk factors and does not have an ANSI approved manager food safety certification. Raw ground beef stored over RTE foods in a prep cooler. Marinara and meat sauce temping 110°F on the steam table. It had not been reheated to above 165°F before hot-holding. Cooked steak that was cooked and began cooling yesterday temped 50°F in the Walk-in cooler. The steak had not cooled quickly and 25 lbs. had to be embargoed. Several cooked foods not date marked. Opened bag of flour on the floor in the outdoor shed. Scoop without handle in parm cheese. Ice spout at drink machine was dirty.

TCS foods must be hot held at 135*F. 1 pound pork embargoed. PIC informed inspector burgers are cooked to order and customer can get them "however they want." When serving foods that are raw or undercooked, a consumer advisory must be present on menu. No consumer advisory provided at time of routine inspection. Showed PIC an example of a consumer advisory from a google search on ipad. Insect activity noted in kitchen area.
