To file a health-related complaint, call the Department of Environmental Health at 423-209-8110. The scores are original and do not reflect on-the-spot changes, which sometimes lead to upward adjustments.

Dishwashwer table and machine installed so dish machine doors can't close. Leak under coke machine creating water build up. Leak at triple sink faucet.

Salad cooler holding cut lettuce at 49°F—moved all salad bags to a working cooler. Not using time as a public health control correctly for sliced tomatoes. Water dripping over single service containers—embargoed 20 containers.
