Reasons include: There was inadequate control over food labeling and storage in the walk-in unit, raw meat was stored in a bin with cream cheese and other ready-to-eat food items, vegetables were stored under meat, raw meat was stored on top of tofu, inadequate date-marking was observed, food boxes were stored on the floor of the walk-in unit, there were uncovered items in the walk-in unit, floor tiles were in poor repair, and employee drinks and personal items were stored intermingled with food and equipment in the front counter area.

Reasons include: The dish-washing employee was washing and rinsing dishes but not submerging dishes into sanitizer at the triple sink, multiple large dishes were in the sanitizer compartment at the triple sink with no room to fully submerge dishes, residual water from dishes in the “clean” area were tested and contained zero sanitizer, multiple food items in the two-door unit were not holding at the proper temperature, the ambient temperature of the two-door reach-in unit was above the proper temperature, the ice scoop was not stored handle up in the ice bin, both deep freezers were not able to close properly because they were overstacked with items and there was a large leak from the piping at the hand-wash sink.
