Reasons include: The person in charge did not demonstrate control over foodborne illness risk factors. The hand sink by the triple sink was used as dirty dish storage. The dishwasher did not have the proper concentration of sanitizer. Rodent droppings were present in many areas of the establishment. The dish pit area was dirty. Pulled pork in the steam well was below the proper temperature for hot-holding. Fried okra was left out at room temperature and was holding below the proper temperature. Burgers held in a metal pan on the side of the hot top were below the proper temperature. The salsa cooler was holding foods above the proper temperature. Numerous cooked animal products were not date-marked in the walk-in cooler. Numerous chemical spray bottles were unlabeled. Numerous flies were present in the dish pit and smoke pit areas. The door to the smoke pit was missing. Foods were stored uncovered in the walk-in cooler. Dirty wipe cloths were stored on the nozzle of an unlabeled spray bottle. Dishes were stored wet-nested. The handle to the reach-in freezer was in poor repair. Lids to dry ingredients were cracked and in poor repair. The top of the dishwasher was dirty. The bottom drawer under the grill top was dirty with food debris. The floor under the dishwasher was dirty and unsealed, and employee drinks were stored on food contact surfaces.

Reasons include: There was no active managerial control over foodborne illness risk factors. The person in charge was unaware of the employee illness policy for reportable symptoms and illnesses. Employees were consuming beverages from unapproved containers in the prep area. Employees were observed not properly washing their hands as needed. There was no running water at the start of the inspection. The hand sink was not accessible for immediate handwashing. Several items were stored in the hand sink, prohibiting immediate handwashing. Raw foods were not stored according to minimum internal cooking temperatures. Raw chicken was stored on top of raw beef in the reach-in cooler. Raw eggs were holding above the proper temperature on the cook line. A thermometer was not locatable in the low-boy unit on the cook line. No probe thermometer was provided for verifying food temperature. Wet wiping cloths were stored on working surfaces and not in sanitizer solution, and numerous non-food contact surfaces were dirty inside the mobile unit.
