Reasons include: Active managerial control was not provided during inspection, an employee consumed a beverage from an unapproved container in the kitchen/drive-thru area, adequate cleaning and sanitizing frequency was not provided for food and non-food contact surfaces, numerous surfaces were dirty throughout the storage, prep and dish-washing areas, there was a strong odor inside the walk-in cooler due to unclean surfaces, the sanitizer at the triple sink was too high, toxic cleaning products were stored on top of the ice cream machine, there was no thermometer inside the small reach-in refrigerator on the make line, clean dishes were stored on dirty surfaces, single-service products were stored on the floor, there was excessive carbon build-up on sheet pans that prohibited proper cleaning, the wash water at the triple sink was below the proper temperature, there were potable water leaks at the mop sink, the toilet in the men’s restroom was not operable, there was grease spillage on the ground around the refuse container, numerous floor tiles were damaged or in poor repair in the dish-washing areas, there was standing water on the damaged floor tiles and there were distressed items in the kitchen, prep and storage areas.

Reasons include: Employee illness agreement was not given and kept for documentation, the employee illness policy was not posted, the person-in-charge did not provide the correct response to illness symptoms, there was no hand soap at the main hand sink, raw eggs were stored above ready-to-eat items in the reach-in cooler, beef and chicken in the warmer were at the improper temperature, several ready-to-eat foods were not labeled with the date prepped/opened, several food items out of their original containers were not labeled, food and single-use items were stored under the hand sink, multiple food items were stored without lids underneath the prep tables, gaskets on multiple coolers had food/dirt debris, the hand sink plumbing was in poor repair, the walls have food/grease accumulation, the vents have dust accumulation, the bathroom ceiling has a small hole in it, there was a flickering light near the hand sink, personal items were stored on top of flour bags and personal food items were stored in restaurant equipment/coolers.

Reasons include: The hand-washing sink at the bar was not working, raw chicken and beef were stored in the top drawer of the cooler above the proper temperature, cut avocados in the prep top were above the proper temperature, a wiping cloth was stored in the reach-in cooler, wiping cloths were stored in sanitizer buckets without sanitizer, a gasket on the draw cooler was in poor repair, the walk-in cooler door was not self-closing, the top cooler drawer was not holding at the proper temperature, there was no trash receptacle at the hand-washing sink and there were gaps between the filters in the hood.
