Employee drink cups found at food stations and in prep areas. Dish machine chlorine was at the wrong concentration. Queso was 89 degrees. Gravy was 113-136 degrees. Ready-to-eat food items were not date labeled. Buttermilk vinaigrette and pudding were kept for over seven days. Chopped lettuce was held in a drawer that was cooled to 52 degrees. The fridge was holding at 48 degrees, when food should be stored at 41 degrees. Multiple gaskets were in poor condition. Walls had excess debris in parts of the kitchen.

An employee touched raw burgers with gloves on, then removed gloves and grabbed bacon out of oven using oven mitts. Then the employee placed the raw burgers on a tray and then went to handle cooked food without washing hands. Dish washer did not reach 160 degrees. Walk-in freezer door was left open and the flooring was detached and coming up. A pipe needs to be extended under an ice bin. A hot water handle at a sink was broken. There was missing floor tile on a set of stairs.
