No sanitizer present in sanitizer bucket at sushi bar. Sushi rice stored at room temperature at 66 degrees Fahrenheit. Rice was not time stamped for time as a public health control. Multiple chemical bottles unlabeled. Debris on knives stored at knife hangers.

Reasons including multiple food workers not washing their hands before beginning food prep. Several workers not washing their hands when entering the kitchen or when changing tasks in the kitchen. Hand washing sink was blocked by boxes in the back kitchen area. Several scoops without handles in food or with handles that were touching the food. Inside of the ice machine was dirty—more regular cleaning required.

Kitchen manager stated they were not reheated properly prior to placing in steam well. Cooked sausage under heat lamp holding at 120F. Food must be hot held at 135F or above. Fly swatters stored on shelf in food storage area. Food debris on top of dishmachine. Food debris and black accumulation from refrigerator gaskets.
