Reasons including person in charge not present at time of inspection; an employee was not aware of reportable symptoms and illnesses with regards to the employee illness policy; another employee observed handling ready-to-eat foods with bare hands. No hand soap was provided at the hand sink by the triple sink; ready-to-eat and time/temperature control for safety foods that were held longer than 24 hours were not date marked. Cleaning products were not properly stored to minimize possible contamination. A cutting board was found to be in poor repair. A buildup of of food debris was found on the pizza oven.

Reasons including employee observed handling ready-to-eat foods with bare hands. Sanitizer was too low at the dish machine, and the sanitizer line was leaking onto the floor. Shelving in the walk-in cooler was rusted, dirty, in poor repair and not easily cleanable. Hot water not operable at the hand sink by the walk-in cooler. Complete separation from laundry services was not provided.

The estimated weight of discarded food was about 7 pounds. Spray bottle was not labeled. Fries were stored soaking in front of the handwashing sink. In-use scoop stored on food prep area. Vent grate fell out of the ceiling and is hanging by wires.
