Reasons including the person in charge not demonstrating control over food-borne illness risk factors at the time of the inspection and a chlorine dishwasher at 0 parts per million instead of 50 parts per million. The inspector had to set up a portion of the double sink as a sanitizing sink and determined that dishes must be washed and rinsed in the dishwasher then dipped in sanitizer water in the double sink, which is not a permanent fix but may be used temporarily until the dishwasher is repaired. Multiple time/temperature control for safety foods were not dated or marked or were held over a seven-day shelf life in cold holding units. Multiple clean product containers contained old date mark stickers. Per kitchen staff, burgers are served undercooked if requested and a consumer advisory was not present on the menu. Insect activity was observed in the chemical storage area and multiple loose screws were stored on top of the dishwasher and handsink. Dirty wiping cloths were stored piled up on the faucet to the double sink. A freezer handle and multiple gaskets were found in poor repair. Test strips were stored on the dishwasher.

Reasons including raw chicken being stored on top of ready-to-eat pineapple slices and rice and beans prepared the day before found in steam well at 88-120 degrees Fahrenheit. Multiple containers of time/temperature control for safety food in the walk-in that were prepared the day before were observed uncovered and without date markings. Mouse droppings were found in the warewash area, on top of chemical buckets and on the floor under the dish machine. Multiple food containers in the walk-in refrigerator were uncovered and the gaskets in the reach-in refrigerator and deep freezer were in poor repair.
