Reasons include: An employee was observed not properly washings hands, raw bacon was stored on top of ready-to-eat deli products in the low-boy unit, raw proteins were stored above ready-to-eat foods in the walk-in cooler, raw chicken wings were sitting out at room temperature holding above the proper temperature, food products were stored on the floor in the walk-in freezer, wet wiping cloths were stored on working surfaces and not in sanitizer solution, in-use utensils were stored between dirty equipment on the cook-line and in the prep areas, there was a cutting board in poor repair on the cook-line, unapproved materials were being used to line the shelving in the walk-in cooler and on the cook-line, no backflow device was provided for the attached hose at the mop sink and the break area was too close to the food prep/storage areas.
