Reasons include: No hand soap was provided in the employee restroom. Raw, hazardous foods were stored above ready-to-eat foods in the low-boy unit. Foods in the glass door cooler were holding above the proper temperature. No thermometer was provided in refrigeration/freezer units. Numerous insects were noted in the prep, dishwashing and storage areas. The nozzle was below the flood level rim at the triple sink. Floors in the dishwashing area were in poor repair, and there was grease accumulation on the hood.
