Reasons include: Active managerial control over foodborne illness risk factors was not demonstrated during inspection. Employees were consuming food items in food preparation areas. Employees were consuming beverages from unapproved containers and stored them on food contact surfaces. Raw foods were stored above ready-to-eat foods and were not stored according to minimum internal cooking temperature in the walk-in cooler. Numerous pans of cooked chicken were cooling at room temperature and were holding at improper temperature. Rodenticides were used and stored haphazardly in the dry storage area. Rodenticide was scattered in dry storage area. There were unlabeled chemicals in the front service area. Proper cooling methods were not provided for cooked chicken on the cook line. Steamed rice was covered and cooling in a large container in the walk-in cooler. Rodent droppings were noted in the dry storage areas. In-use utensils were stored in standing water. Single-service cardboard boxes were reused for food contact containers in the walk-in cooler. Stoppers at the sanitizing compartment of the triple sink were in poor repair and were not adequate. The ice chute was dirty at the drive-through and at the customer self-service machine, and employee personal items were not properly stored in the designated areas.

Reasons include: An employee on the cook line was handling ready-to-eat foods with bare hands. Hand soap at the hand sink by the dish-washing area could not be dispensed. Raw foods were stored above ready-to-eat foods in the walk-in cooler. Raw foods were not stored according to minimum internal cooking temperatures. Beef livers were hot holding below the proper temperature at the cook line. Insect activity was noted in the dish-washing area and at the wait station. There were uncovered/unprotected food items in the walk-in cooler. Food items were stored on the floor in the walk-in freezer and cooler, and wet wiping cloths were stored on working surfaces and not in sanitizer solution.

Rice was cooling with a lid on tightly. There was an uncovered flour bin in the raw cook area. Wet wiping cloths were stored in plain water instead of sanitizer. Pans were nesting wet. There was some buildup on pans in the clean storage area. Floor tiles were in poor repair, and there was an employee drink over the prep area.
