Reasons include: Person-in-charge did not demonstrate active managerial control. A staff member cracked a raw egg, touched a bowl that was handed back to the cold-bar cook and then placed cooked food on a plate without washing hands or changing gloves. Dish worker prepared dirty dishes and then unloaded clean dishes without changing gloves or washing hands. Cook was chopping cilantro with bare hands. Raw chicken was stored over raw vegetables in the walk-in cooler. Raw chicken was stored over potatoes in the refrigerator. Shredded carrots were holding above proper temperature. No date-marking of any food items was observed in the kitchen or coolers. Boxes of frozen seafood, carrots and peas were thawing at room temperature. Numerous foods and sauces were stored out of their original containers and in unlabeled plastic ice cream containers. Box of peas and carrots was thawing directly on the floor under a shelf. Strainers of shredded carrots were stored on top of carbon dioxide canisters. Food was stored on the floor of the walk-in freezer. A large, round wooden dish used for sushi rice had brown buildup around the perimeter. Multiple scoops were stored down in food product. Ice scoop was stored directly on dirty ice machine surface. Plates and bowls were being wet-stacked. Raw wood was being used to line shelves in the dry storage room. Top and inside of the ice machine were dirty with brown buildup and debris, and the water canister used for the cook line had dark buildup.

Reasons include: An employee partially consumed food products stored in the reach-in cooler with food items for customers. Raw animal products were stored on top of and above ready-to-eat foods in the reach-in refrigerator. No date-marking was provided for ready-to-eat, time-sensitive foods. An in-use utensil was stored with its handle in raw product on the cook line cooling drawers. The ceiling was not properly constructed in the kitchen to provide a smooth, cleanable surface, and employee personal items were not properly stored in designated areas.

Reasons include: An employee consumed a personal beverage from an unapproved container while directly preparing food. Sanitizer was too strong at the triple sink. There were uncovered/unprotected food items in the walk-in cooler. Wet wiping cloths were stored on working surfaces and not in sanitizer solution. Clean utensils and dishes were stored on dirty surfaces. Door gaskets were in poor repair on the reach-in cooler on the cook line. Nonfood contact surfaces were dirty on the cook line and in the food prep area. The ice machine was dirty inside at the deflector panel. The floors were dirty behind and underneath equipment on the cook line, and employee personal items were not properly stored in designated areas.
