Reasons include: Active managerial control over foodborne illness risk factors was not provided. The person in charge was unaware of the employee illness policy for reportable symptoms and illnesses. No hand soap was provided at the designated handwashing sinks. There were severely dented canned goods in the dry storage area. Foods in the walk-in cooler were holding above the proper temperature. Sanitizer at the triple sink was at the improper level. There were uncovered/unprotected food items in the dry storage area. The door gaskets on the refrigeration units in the prep area were in poor repair. There was a buildup of food debris on the pizza oven rollers. Cold water at the triple sink was not operable, and the hood filters and hood were dirty over the pizza oven.

A utensil was stored with its handle in product, and employee personal items and beverages were stored on food prep surfaces.

The glass reach-in cooler door did not close all the way, and multiple nonfood contact surfaces in the kitchen were dirty.
