Reasons include: Management was not in control of foodborne illness factors. There were multiple adulterated onions. Dish machine had no sanitizer. Sanitizer bottle was empty. Multiple food items were not date-marked and were past seven days since preparation. There were multiple unlabeled spray bottles. There were multiple large tubs of lamb and fritters cooling on the counter. Multiple foods were uncovered in cold storage. Multiple boxes of food were stored on the floor in the walk-in freezer. The ice machine was dirty, and the ceiling was in poor cleanliness.

Reasons include: Person-in-charge lacked managerial control over food safety. The only functioning hand sink in the kitchen was blocked by two carts. The back hand sink by the dish room was not functioning. Hot food items at the buffet were holding below proper temperature. Cold food items were holding above proper temperatures. Multiple items in the tall reach-in cooler by the cook line did not have date-marking. Spray bottle labeled “Windex” at the server station contained an orange liquid. Gaps were present along both outer back doors near the dish room. Food items were not covered in the walk-in cooler. Wiping cloths were being stored on counters of the buffet. Rice scoops at the buffet were stored in tepid water. Cardboard was used to line shelving throughout the kitchen. The walk-in cooler door was in poor repair. The standing freezer was in poor repair with a cracked lid that exposed insulation material. There was grease build-up on surfaces around the grill area. The dumpster and surrounding area were poorly maintained, with broken equipment and spilled liquid refuse. Floors throughout the kitchen were in poor repair. The walls by the back door were in poor repair, and the light was not working in the back area by the dish room.

Raw chicken breasts were stored above raw bacon in the reach-in cooler. Multiple sanitizer buckets had no concentration of sanitizer, and multiple foods were held below the proper temperature, including pinto beans, mashed potatoes and cooked apples.
