Reasons include: Personal blankets/covers/bedding items were stored on shelving in the dry storage area. Employees were not practicing double hand-washing after returning from the restroom. Meatballs were reheated and holding below the proper temperature. Foods were thawing at room temperature. There was a ceiling leak in the dry storage area. There was excessive litter in the parking lot. A complete separation from living/sleeping quarters was not provided, and the lighting was not adequate in the walk-in cooler and freezer.

Reasons include: Raw foods were not stored according to minimum internal cooking temperatures in the walk-in cooler. Fried chicken under the heat lamp was holding below the proper temperature. Employee hair was not properly and adequately restrained. Wet wiping cloths were stored on working surfaces and not in sanitizer solution. There was no test kit for chlorine sanitizer at the triple sink, and floor tiles were in poor repair on the cook line and in the storage area.
