Reasons include: Person-in-charge does not have active managerial control over food safety, a cook cracked raw shell eggs with gloves on and then grabbed clean utensils without washing hands or changing gloves, cook handled bags of raw meat and then handled clean utensils without washing hands in between, raw steak was stored above ready-to-eat foods in the two-door unit, cook “washed” a pan in the triple sink and placed it back on the cook top without proper sanitizing, rice on the sushi prep line was holding above proper temperature and had not been date-marked, raw beef and chicken were thawing at room temperature, squeeze bottles containing food products were not properly labeled, there was a significant mouse infestation, mouse droppings were observed under and on top of the dish machine, on top of canned goods and bulk food storage and under all equipment, there were open bags of bulk food products, there was food debris on bulk storage bin lids, walls contained multiple holes, there was food debris on the walls of the prep areas, there were cracks in the concrete flooring with standing water in them and an employee drink was not stored in the designated area.
