Reasons include: Person-in-charge was not in active control of kitchen, an employee bare-handed cilantro onto ready-to-eat food, the facility was in poor cleanliness and repair, multiple food and non-food contact surfaces needed to be thoroughly cleaned, multiple pans of chicken prepared the day before had not cooled to the proper temperature in the prep-top and low-boy unit, multiple food items were not properly labeled, there were rodent droppings in the light shield, there were insects in the kitchen, multiple boxes of chicken were stored on the floor, multiple gaskets were in poor repair, the freezer had heavy ice build-up, hot water was not available at the hand sink and the sprayer sink and air conditioning were leaking.
