Reasons include: Active managerial control over foodborne illness risk factors was not observed. Employees were consuming personal beverages from unapproved containers on the prep line. A personal beverage with no lid and straw provided was stored on the prep line too close to food items. There was not adequate handwashing by employees while changing tasks. Chicken patties were holding below proper temperature on the cook line. Liquid egg product was holding above proper temperature in an ice bath on the cook line. There were uncovered/unprotected food items in the walk-in cooler. There were some nonfood contact surfaces that were dirty on the cook and prep lines. The convection oven was dirty inside, and numerous maintenance equipment was stored haphazardly in the mop/utility sink.

Improper cooling methods were being used in the walk-in cooler, and soap solution was not dispensing at the dish machine.

Some ready-to-eat foods held longer than 24 hours were not properly date-marked in the walk-in cooler. Food products were stored on the floor in the walk-in freezer and in the dry storage area. Soiled wiping cloths were stored on working surfaces and not in sanitizer solution. The can opener blade area and base were dirty. The floors in the walk-in freezer were dirty, and there were distressed items in the kitchen and storage areas.
