Numerous food and nonfood contact surfaces were dirty in the prep area and throughout the establishment. Numerous dairy products were left out at room temperature. Numerous dairy products were above the proper temperature. Food items from delivery were stored outside the establishment on a guest table. Multiple dirty wiping rags were stored on food contact surfaces in the prep area. The ice scoop was stored embedded in ice. There were no sanitizer test strips available, Numerous areas of the food prep area were dirty with spilt dairy products. There were multiple empty cans on the floor. There were overflowing garbage cans. The floor was dirty with dairy spills, excessive water and ice, empty cans and overflowing garbage, and numerous employee foods, drinks and clothing were stored with food products in multiple areas of the establishment.

Nonfood contact surfaces were dirty on the make line. The plate chiller was dirty inside. There was a buildup of food debris on the rear portions of the deep fryers. The dipper well inlet was below the flood level rim. The refuse area was dirty around the dumpsters. The floors were dirty behind/underneath the convection ovens, and the walls were dirty underneath the scraping sink in the dish-washing area.
