Reasons include: There were multiple dented cans, raw chicken, raw chorizo and raw beef were in the low-boy reach-in unit next to each other with no covering, the triple sink and sanitizer bucket had sanitizer concentration below the proper level, queso’s internal temperature was below the proper temperature, food items were stored on the ground in a container, the hand sink had a constant drip of water from the faucet, the ice machine drain pipe was in poor repair, the women’s restroom had no enclosed garbage can and an employee drink was on the prep table.

Reasons include: Person-in-charge did not have managerial control over foodborne risk factors, the hand sink was filled with utensils and employee personal items, cluttered hand sink prevented employees from properly washing hands, multiple ready-to-eat foods held more than 24 hours in the low-boy and reach-in coolers were not date-marked, raw chicken from delivery was stored on the ground, cardboard was used on a wooden bench to hold utensils and food during and after the cooking process, cardboard was used for food storage in the reach-in cooler, multiple non-food contact surfaces in the kitchen area had a build-up of grime and the floors, walls and ceilings had accumulations of grease and grime.
