Reasons include: No person in the kitchen had active managerial control over food safety. A cook breaded raw chicken and then took off one of two gloves to handle ready-to-eat food and clean dishes without washing hands in between tasks. An employee was chopping cabbage without gloves. Chicken soup prepared the day before and contained in a large stock pot was holding above the proper temperature in the two-door tall unit. There was an unlabeled pesticide sprayer in the kitchen, and dishes were stacked wet.

Reasons include: There was no food safety certification posted. The person in charge had no employee health documents or acknowledgement of food handler staff’s responsibility to report diseases transmissible through food. The microwave roof had excess soil accumulation. Two reach-in coolers and the walk-in cooler were in poor repair, and the handles on the majority of the reach-in coolers had a sticky residue on them.
