Reasons included inadequate control over multiple food safety risk factors; cigarette butts in various containers around kitchen; handling raw fish and changing gloves without washing hands; kitchen hand sink being used to thaw fish; mac and cheese holding at 120 F, and baked potatoes holding at 106 F, reheated to 165 F; inadequate date marking throughout kitchen. Inspector observed cook rinsing gloves rather than changing them to wash hands; multiple food and non-food contact surfaces in poor repair; multiple food and non-food contact surfaces dirty excessive trash build up around building; and multiple employee drink cups in prep area.

Reasons included inspector observed employee enter the building, clock in and proceed to don gloves without washing hands first; observed ziplock bag of raw chicken in walk in unit stored on top of box of tomatoes; beans, steak holding at 103 F, 118 F in unit, also cooked chicken in hot box holding at 122 F, these items were reheated during inspection to 165 F for proper hot holding at 135 F; cooked chicken wings stored at bottom of slider unit not covered; scoop for rice stored in tap water at 66 F; and employee protein shake observed in two-door slider unit along with retail food sale storage.
