Reasons include: Person in charge did not demonstrate control over foodborne illness risk factors at the time of inspection; inspector did not observe hand-washing by food prep employees throughout duration of inspection; numerous food and non-food contact surfaces were dirty throughout the establishment; adequate cleaning and sanitizing frequency was not provided; uncooked pizzas were stored on timer rack as procedures were not being used; triple sink sanitizer exceeded proper levels; thermometers were not present in multiple cold-holding units; tools, glue, etc. stored above pizza dough on storage rack; walk-in cooler door was in poor repair; dish rack was chipping and in poor repair; sanitizer test strips were not available at triple sink; there was excessive grime build-up on triple sink nozzles and handles; food racks had excessive grime build-up; bottom of prep cooler was dirty; floors were dirty under multiple areas of restaurant, including employee table, single service racks, chemical racks, walk-in cooler racks, triple sink, dish rack, etc.; walls were dusty around pizza oven and back side of walk-in cooler; there was a strong foul odor coming from drain of triple sink area.
