Reasons include: An employee returned from the restroom and did not wash their hands. Raw foods in the low boy unit were stored too close to cooked and ready-to-eat foods. Food products were stored too close to the floor in the walk-in cooler. Wet wiping cloths were stored on working surfaces and not in sanitizer solution. An in-use utensil was stored in standing water at the rice station. Duct tape was utilized on handles of deep fryer baskets. Only one drainboard was provided for the triple sink. Some nonfood contact surfaces were dirty on the cook line. The ice machine was dirty inside at the deflector panel. Floors were dirty behind and underneath equipment. There was litter in the parking lot, and designated areas for employee personal items were not utilized.
