Reasons include: The manager did not have control over foodborne illness risk factors; there was no hand-washing by any employees during the entire inspection; employees did not wash their hands when entering the kitchen from outside; the hand sink by the drink machine was blocked and did not have paper towels; there was no hot water at the hand sink at the bar; tapioca boba balls were hot holding at improper temperature; several foods in the walk-in cooler that were prepared several days ago were not date-marked; there was an unlabeled chemical spray bottle on a cart; staff did not have a probe thermometer to take food temperatures; the back door was left open and was in poor repair; multiple food items were being stored on the floor of the walk-in, freezer and dry storage rooms; wet wiping cloths were stored on prep surfaces; bowls without handles are being used to scoop dry ingredients; single-use items were being stored on the floor of the storage closet; multiple storage containers were in poor repair with cracking and chipping; wire shelving in the walk-in was rusty and in poor repair; cardboard was being used in the cold bar to divide vegetables; the outside of multiple food containers were dirty; there was an uncovered employee drink on the food prep surface; there was no health inspection posted in the establishment.
