Reasons include: The person-in-charge did not have control of foodborne illness factors; pepperoni and cheese were stored on a table top above proper temperature; cooked chicken wings and pepperoni were not date-marked; sanitizer was above proper level; fruit flies were present in the mop sink area; multiple non-food contact surfaces were dirty; and the ceiling and floors were in poor cleanliness.

Reasons include: A cook rinsed gloved hands under water at the prep sink, touched a wet, dirty cloth on the counter and then went back to food prep; there were dirty cups and a dish-scrubbing device in the hand sink; raw chicken wings were stored over raw sprouts in the back of the reach-in cooler, and there was raw chicken stored overcooked white rice in the low-boy coolers; bins of raw chicken were being placed on top of the cutting board at the cold bar; a sanitizer bucket had soapy water without sanitizer; wet cloths from the sanitizer-less bucket were being stored on prep surfaces; cutting boards were worn and stained; the refrigerator was dirty with food debris on the inside and outside; and the ice maker was dirty inside and on the lid.
