Multiple foods in the low-boy cooler were holding above the proper temperature. Low-boy cooler had an ambient temperature above the proper level, and employee beverages were stored on the food prep surface.

Multiple fruit flies were present in the rear area of the establishment. Multiple smoothie powders were stored without lids in the prep area. The bottom of the reach-in freezer was dirty with food debris. The triple sink chemical dispenser was spewing from the side of the unit. The hand sink in the front area had a constant water drip. Employee drinks were stored on food contact surfaces near food products, and employee personal items were stored embedded with food products on the dry storage rack.
