Reasons include: Active managerial control over foodborne illness risk factors was not provided. Seasoning mix was not of sound condition (signs of moisture contaminating product) in the dry storage area. Sausage was hot, holding below the proper temperature. Cole slaw holding in the glass-door cooler was above the proper temperature. Cut tomatoes were holding above the proper temperature in the sandwich cooler on the cook line. No thermometer was provided in the sandwich low-boy cooler on the cook line. Bulk drinks were stored in the edible ice bin at the drive-through. The in-use bulk ice scoop was stored with its handle in product at the drive-through. The triple sink compartments were soiled/dirty. The wash solution at the triple sink was dirty. There were several missing floor tiles in the kitchen causing water to pool on the floors, and the walls were dirty in the walk-in cooler.

There was a gray water leak at the mop sink/condensate drain, and employee personal items were not properly stored in designated areas.
