When a failure to maintain proper heat holding was discussed and corrective actions explained, after approximately 30 minutes, the violation remained, and corrective action had to be explained again. Observed employee eating lunch using hand to mouth at the cook line in the kitchen. Kitchen hand-wash sink observed with debris and dust accumulation and clearly has not been used. One employee did not know how to use the paper towel dispenser, and one employee tried to use the chemical at the mop sink as soap (this was corrected). Raw chicken observed in refrigerator stored above prepared meat patties. Two pots of black beans that the person in charge stated were prepared the previous day were holding in refrigerator at 53 degrees and 60 degrees. Pots were filled and not cooling properly. Cooked chicken heat holding at front serving line at 112 degrees. Ready-to-eat items, such as patties and beans, were not properly date marked in the kitchen. Techniques to ensure proper cooling were not implemented. Observed employee prepping oxtail in sink compartment next to "clean" dishes on drainboard of sink. Repair needed on refrigerator handle to ensure smooth and easily cleanable handle. Gaskets in refrigerator were in poor repair. Ceiling was in poor repair, and portions were covered with cardboard. An open employee drink was found on the food prep counter.

The person in charge also stated they had not been trained on food safety as they just started working at facility. Observed person in charge enter facility and, without washing hands, put on gloves to prepare food. The person in charge stated they used hand sanitizer. Both hand-wash sinks in facility do not have soap dispensers that operate. Two containers of meatballs observed in warmer holding at 66 degrees and 56 degrees. The person in charge was unfamiliar with reheating requirements for heat holding. Utensils stored in standing water. Lid not properly attached on ice machine. Gaskets in prep refrigerator were in poor repair. Plumbing leak found at back hand-wash sink.
