Reasons include: An employee was observed handling ready-to-eat foods without gloves on; required date marking was inadequate; cook and employees on-site stated burgers are sold undercooked (medium rare, medium well), but there is no consumer advisory posted in facility or on menus; multiple spray bottles in the kitchen were unlabeled, and one spray bottle of degreaser had its stream pointed at clean dish storage; there was standing water in the bottom of a two-door reach-in unit; there was a bag of food stored on the floor; knives were stored in between equipment; utensils were stored in standing water; gaskets were in poor repair at the two-door reach-in unit; cardboard was used to line the floor; there was accumulated debris at the potato slicer, microwave handle and flooring in front of the three compartment sink.
